Thank you for joining the PLAY team! To ensure the safety of our youth and the legal health of our charity, please complete the following onboarding steps. Most mentors finish this process in under 30 minutes.
Before beginning, download both essential documents. We recommend saving these to a dedicated folder on your computer for easy access.
Mentor Policy Handbook
Content: Code of Conduct, Confidentiality, Youth Safety, and Communication Guidelines.
Action: Read all 10 pages carefully. You are only required to submit the signed final page (Page 10).
Mentor Application Form
Content: Personal Information, Background, Screening, and References.
Action: Complete all four pages of the application in full.
All forms are provided as fillable PDFs. You may complete these on your device without the need for printing.
Complete All Fields: Ensure all text boxes are filled and the checkboxes in the Screening and Declaration sections are marked.
Signatures Required: 1. Application Form: Provide a signature on Page 4. 2. Policy Handbook: Provide a signature on Page 10 (The Agreement Statement).
Digital Tools: You may use the "Fill and Sign" tool in Adobe Acrobat or the "Markup" tool on your device to apply your digital signature.
As part of our commitment to youth safety, all mentors must provide a valid Vulnerable Sector Check from their local police service.
Existing Check: If you have a VSC completed within the last 6 months, you may submit a clear digital scan or photo of the document.
New Check (Ottawa Residents): You can apply online through the Ottawa Police Service. Use the button below to start your application.
Volunteer Letter: To avoid paying the full employment fee, you will need a volunteer letter from us. Please email info@playouth.org with the subject "Volunteer Letter Request" before you apply.
Filename: Save your final certificate as: Lastname_Firstname_VSC.
To ensure all PLAY mentors provide an inclusive environment, we require the completion of the Ontario AODA (Accessibility for Ontarians with Disabilities Act) training. This is a provincial requirement for all volunteers.
Action: Complete the "Customer Service Standard" and "Human Rights" modules.
Time: This free training takes approximately 45-60 minutes to complete.
Certificate: You will receive a PDF certificate upon completion. Please save this file to your device.
To complete your application, please rename and submit your files according to the instructions below.

Verify Data: Open your renamed files to ensure all signatures and entered information have been saved correctly.
Submit via Email: Attach all four documents and send them to info@playouth.ca.

Once we receive your documents, our Program Coordinator will review them within 48 hours.
If everything is complete: You will receive a mail to book your Onboarding Interview.
If items are missing: We will reply to your email letting you know what needs to be fixed.
